The only time I've seen free air to that extent was with a trachea perf from a bad intubation in the field but most of the free air was subcutaneous n we darted.

If you don't RSI right or if the person is a difficult airway its a risk. That situation was a girl who'd been doped up n gang raped then left for dead in a stash house for getting on the wrong side of her ex n his shithead friends. Found down after someone got a conscious n reported it.
Pretty sure she was hypoxic at that point but rescue Ricky was gonna save her goddammit n since he thought no gag meant no reason to paralyze he just shoved it in!!! Again. And again.
And again.... They finally dropped an LMA n she earned herself an emergency trach in the ER (perks of being dragged to a lvl 1 with decent surgeons) but they had pumped so much air into her that she swelled up like the Michelin man.

Well Tim I believe her demographic info listed her as white. So was ths perpetrator of the incident.
She was a middle upperclass kid from NYC from a really sweet family. She was dumped on our side of the Delaware in PA cuz thats what unpleasant ppl do when they wanna dispose of things.
Idk why this kinda stuff happens to good people, but it does. It's why I come here to laugh n shitpost.

Here's a confirmed spontaneous bowel perf with air demonstrated under the diaphragm on a cxr
