Why do images not work properly?

Every time I try to use the file upload to add an image to a post it will never appear despite the file upload succeeding, you need to use an external host to actually have it work. The file will show up in the editor view but fail once its published. Admins fix this

You most likely can not view the image above ^ despite it successfully embedding.

I embed the file and it uploads to the servers but it just doesn't appear when published??? Xenforo is a mysterious lady.
