I can't imagine shitting in the shower working out very well, you'd have to like squat down in there and I don't see it working out. Much harder than taking a simple piss.

Prevalent but not legally required. Even in Italy it is just a requirement in the building code, and you can get exceptions quite easily. Most builders do not apply for these because bidets are popular and the additional cost and space requirements are minor.

The problem is the size of the drain hole. If I have to manually push a turd down the shower pipe, then any potential convienence or advantage of taking a shit in the shower has been eliminated. It's just easier to use the toilet, but only to then hop back into the shower instead of wiping. THAT'S more convenient.

On the other hand, if shower drains looked like this, then it'd be a different story:
